Primary care home visits

Patients that fit the following criteria are eligible for home care.

• Patients are 65 years old or older
• Live in the Morris County area
• In need of care of primary care
• Homebound - patient has to exert considerable and taxing effort to leave the home due to illness or injury
• Requires assistive device to leave residence
• Use of special transportation to leave residence
• Assistance of another person to leave residence
• Has medical contraindications to leaving the house

Our home care programs offer numerous benefits for elderly patients who are homebound. Some key advantages include:

• Personalized care to ensure patients receive the exact level of care and attention they require
• Safety
• Collaborative approach that involves family members in the caregiving process

• When your appointment has been scheduled - you will receive a confirmation text and/or call confirming the date and time of your appointment.
• A week before your appointment - you will receive a confirmation text and/or call through our automated system.
• The day of your appointment - expect your scheduled provider to arrive within a 30 minute window of your appointment time. Please have your phone available as our office may reach out to you to communicate any delay’s or early arrival.
• If bloodwork or imaging is needed - you will be receiving a call from one of our affiliated companies to schedule. 
• The close of your visit - expect a call to schedule your follow up appointment.
• A copy of your visit and orders will be shared with you via MyChart for your records.
